Lactoferrin supplements: what's in the bottle

Where it comes from, the words you'll see on a label, and the one dose with trial results behind it.

Short answer

A lactoferrin supplement contains a protein found in milk. The trials with the clearest results used lactoferrin from cow's milk, 100 mg twice a day, in pregnant women with iron deficiency anemia. Those trials don't tell you what it does for anything else. The FDA doesn't check whether a supplement works before it goes on sale. If you're allergic to milk protein, skip it.

You're comparing bottles and trying to work out what you'd actually be buying. This page covers where the lactoferrin in a supplement comes from, the words printed on labels, and the dose the trials used. It won't tell you which product to buy, and nothing here is for sale.

It's a milk protein

The lactoferrin in the trials below was bovine, meaning it came from cow's milk. Labels tend to present it as an immune ingredient, so its dairy origin is easy to miss.

Milk is one of nine major food allergens under US law, and the FDA's allergen labeling rules cover dietary supplements. A product made with bovine lactoferrin has to declare milk. If you have a milk protein allergy, that is the line to look for.

There is also a newer kind made without cows. A US company, Helaina, has engineered yeast to make lactoferrin with the same protein sequence as the lactoferrin in human breast milk. Its own scientists analyzed it in 2024. The protein chain matched, and its shape came close, but the sugar chains attached to it were different. That was lab chemistry, run by the maker, and it did not test the protein in people.

The dose the trials used

In a 2009 trial in Naples, Italy, 100 pregnant women with iron deficiency anemia took one of two treatments. Half took 100 mg of bovine lactoferrin twice a day. The other half took 520 mg of ferrous sulfate, a standard iron pill, once a day.

After 30 days, both groups had higher hemoglobin (the iron-carrying protein in red blood cells) and better iron stores. The study found no clear difference between the two groups. The women taking lactoferrin reported less stomach pain and less constipation.

A 2017 review pooled four trials covering 600 pregnant women and came out in favor of lactoferrin. Look closer, though. In the three trials of mild anemia, there was no clear difference from iron pills. The clear gain came from a single trial of 228 women with moderate anemia.

So 200 mg a day is the amount with trial results behind it, in one group of people, for one purpose. If a bottle gives a very different amount per serving, it's fair to ask the maker what evidence they used. If you're pregnant, treating anemia is a decision for you and your doctor or midwife.

Apo and holo

These words describe how much iron the lactoferrin is carrying. Apo-lactoferrin, sometimes written apolactoferrin, carries almost none: under 5% of what it can hold. Holo-lactoferrin is loaded, at over 85%. A 2025 review found that makers and researchers often overlook the difference, partly because there is no standard method for measuring it.

A search of PubMed for randomized trials that name both forms found one, in September 2026, and it was in babies. In 2020, 25 infants in Kenya ate a single porridge meal with each kind. They absorbed more iron when apo-lactoferrin was added to an iron salt. Iron from holo-lactoferrin was absorbed about as well as iron from ferrous sulfate.

That study measured iron absorption from one meal in infants. It can't show that either form does more for an adult taking a capsule every day, so treat a label that promises otherwise with suspicion.

What the label shows, and what it leaves out

The FDA isn't allowed to approve supplements for safety or effectiveness before they go on sale. The label follows FDA rules, though, and a few parts repay a close read:

  • The "Daily Value not established" footnote. Lactoferrin has no Daily Value, so a symbol in that column points to this line. It means no official daily amount exists to compare against.
  • A proprietary blend. If lactoferrin is one of several ingredients in a named blend, the label only has to give the blend's total weight, with ingredients listed from most to least. You can't tell how much of that weight is lactoferrin.
  • The allergen statement. Look for milk.
  • The words "bovine", "apo" or "holo". These tell you the source and the iron load, when the maker chooses to state them.

No label can tell you whether a product will help with the reason you're buying it. The trial evidence on this page covers anemia in pregnancy. For any other claim, look for a named study and check what it measured.

Check with a doctor first if

You're pregnant or breastfeeding, you have a condition that affects your iron levels, or you're thinking of giving it to a child. Lactoferrin binds iron, and iron was what the trials above measured. And if you have a milk protein allergy, don't take bovine lactoferrin at all.
