Lactoferrin supplements: reading the label
What is actually in the bottle, what the numbers mean, and who should skip it.
Short answer
Lactoferrin supplements are made from cow’s milk or whey. The trials behind the strongest claim used 100 mg twice a day. The research behind them is genuinely mixed: reasonable for iron deficiency, thin or absent for most of the other claims. If you are allergic to milk protein, this is not for you.
Where it comes from
Effectively all supplemental lactoferrin is bovine, separated out of cow’s milk or from the whey left over from cheese making. Human and bovine lactoferrin are similar but not identical, which is one reason results in adults do not always follow from studies of breastfed infants.
This also means it is a dairy product. If you have a cow’s milk protein allergy, avoid it. That is the clearest rule on this page and the one most often missed, because lactoferrin tends to be marketed as an immune ingredient rather than as a milk protein.
How much did the research actually use
This is a better question than what a given bottle contains, because bottles vary and trials are written down. The pregnancy trials that produced the strongest evidence for lactoferrin gave 100 mg of bovine lactoferrin twice a day, so 200 mg daily, compared against 520 mg of ferrous sulfate once a day.
If a product's serving size is far from that, it is worth asking what evidence the maker is working from. And compare cost per gram rather than price per bottle, since serving sizes differ enough to make sticker prices meaningless.
Apo and holo, and whether it matters
Lactoferrin is sold in two states. Apo-lactoferrin carries little or no iron. Holo-lactoferrin is largely saturated with it. Some labels make a great deal of this distinction.
The honest position is that the difference is real chemically, and that evidence showing it changes outcomes in people is limited. Treat confident claims in either direction with suspicion.
What is worth comparing
- Milligrams per serving, and servings per bottle. Cost per gram is the only fair comparison, and it is rarely the one on display.
- Whether the number is lactoferrin or total protein. These are not the same and some labels blur them.
- Purity, if stated. Products vary in how much other milk protein comes along with it.
- Whether it is enteric coated. Some products aim to survive stomach acid. Whether that improves anything measurable is not well established.
Who should be careful
Anyone with a milk protein allergy should avoid it entirely. Anyone managing an iron disorder in either direction should raise it with a doctor first, since binding iron is the whole mechanism. If you are pregnant, breastfeeding or considering giving it to a child, that is a conversation with a clinician rather than a decision to make from a product page.
